The Indgenous Outreach Team is here to help!

Out team, in partnership with NATSIHA, is aiming to close the gap when it comes to chronic disease within the Indigenous community. We are also trained to assist you in accessing and registering for My Health Record, a government health initiative.

Heart Disease
Liver Disease
Kidney Diesase

We provide assistanct to the community with:
Travel and attendance to GP, diagnostic and specialist appointments throughout the region
Collection of medication
Advocacy and support through primary health care visits
Referral to other agencies for Chronic Disease support
Assistance with filling out health related forms
